Horizon Health & Subacute Center Frequently Asked QuestionsHorizon Health & Subacute Center Horizon Health & Subacute Center

1. What should I bring with me?

Four to seven changes of comfortable loose fitting clothing, including undergarments; pajamas, nightgowns, and a robe, if desired; comfortable non-skid footwear; cosmetics or toiletries of choice (especially if you have sensitive skin); leisure-time materials (books, music cd’s, crossword puzzles, etc) of your interest. Please make sure that all of your personal items are properly labeled and added to your inventory sheet.

2. Is there anything I should NOT bring?

Medications from home, including over the counter medications UNLESS specifically requested by your nurse; items of extreme sentimental or monetary value; cash, checkbooks, or credit cards. Space is limited, so if you choose to bring in some small furnishings, pictures, etc. from home, please check with our Maintenance Department.

3. What are the visiting hours?

For the comfort and security of our guests, we try to encourage visiting between 10 am and 8 pm; however you may have visitors at any time and families with critically ill guests in residence are encouraged to spend as much time as necessary for your own comfort at such a difficult time.

4. What if I need transportation to an outside appointment?

Our Medical Records Associates will gladly explain your options and make the arrangements you decide upon, either with our in-house transporters if available or with a community provider. Transportation costs are generally NOT covered by insurance and will be an out of pocket expense.

5. When are meals served?

Breakfast is generally served between 7-8:30 am, Lunch from 12-1:30 pm, and Dinner from 5-6:30 pm. You may choose to eat in your room or to visit our lovely dining room for all three meals.

6. Can I leave the building to go out with my family for dinner or a visit?

Short outings are often very useful as part of your recuperation. They lift your spirits and therefore your emotional and physical well-being. Your insurance coverage does have some say in how long and how frequent you can be absent from your inpatient stay and still qualify for your coverage, so please ask your nurse or Social Services designee.

7. How will I know what the plan for my stay and care is?

You and, if you’d like, your interested family member, will be invited to a care plan conference within the first week of your stay. This short yet informative meeting is designed to review your doctor’s current orders and those treatments you are receiving which is why you are here in the first place. You will be updated weekly by your discharge planner as to how well you are progressing towards your goal to return to the community. If our care plan conference time is inconvenient for you or your family member to attend, we will try to make other arrangements with you so that your questions will be answered and you will be informed.

8. Can children and grandchildren visit?

Of course! Please enjoy special time with your young visitors.  While we welcome kids of all ages with a smile, we ask that children too sick to attend school or daycare not come to visit (especially during cold and flu season) and that all children must be supervised at all times.
